Transcript Request

Mercy College of Ohio’s Student Records Office maintains all of the student records forMercy College of Northwest Ohio, Mercy School of Nursing, St. Vincent School of Nursing, St. Vincent School of Radiography, and the College since its inception in 1993.

To request a transcript for any of the above institutions, you must put the request in writing. To do so, you must complete a Transcript Request Form.

You can either:

1. Submit it in person on the 5^th Floor of the College,
2. Fax it to 419-251-0629 attn: Peggy Zeller, or
3. Mail it to: Attn: Peggy Zeller, 2221 Madison Ave., Toledo, OH

All transcript requests are completed in 24-48 hours upon receipt. Also, there are no fees associated with transcript requests.
